Elm House is a licensed Adult Foster Care home which provides training, care and supervision to fifteen adults with developmental disabilities, mental illness or traumatic brain injuries, in a twenty-four hour supervised home. Elm House is operated by HOME, Inc and is a non profit 501(c)3. The home was opened in 1978 and has been successful in supporting individuals to enjoy a thriving life, prepare individual who are capable for supported independent living and facilitate participation in outside programming and work opportunities.
Elm House was built through a loan from the Michigan State Housing Development Authority (MSHDA) and a gift of land from SSIHM. Most residents are eligible for HUD Section 8 Housing Subsidy. Additional funding is available through Social Security/Supplemental Security Income, The Michigan Department of Health and Human Services and Bridge Card. Elm House also accepts individuals on a private pay basis. Elm House residents live two to a room and share a bathroom with two other residents (Dorm style). The home features two large common areas and a main kitchen and dining room. There is space to play games, create art, watch TV and more. Residents of the home are welcome to have friends and family visit and participate in daily activities. Residents of the home are typically involved in a number of social and recreational opportunities such as day programs, Special Olympics, bowling leagues, movies and dining out. Community Supported Living Arrangements HOME, Inc. has long been a leader in the development of supported independent living programs. Community Supported Living Arrangements (CSLA) strives to provide whatever services and supports and individual needs to maintain him/herself in the community. For some this may be a few hours a month of very specific supports. For others, it may require nearly twenty-four hours per day of supports and supervision. The level of support is determined by the needs specified in an Individual Plan of Service which is developed by the client, his/her family, and other agencies serving the individual. The provision of services in the CSLA program is monitored by the Monroe Community Mental Health Authority, the primary funding agency, and through the use of Consumer and Family/Advocate Satisfaction Surveys. Our CSLA program has received accreditation from Community Health Accreditation Partners (CHAP). Clients served by the CSLA program often need assistance with budgeting and money handling, menu planning, meal preparation, cleaning, medications, medical appointments, and transportation. Some individuals need more intense supervision or assistance with personal supports such as bathing, dressing, and personal hygiene. HOME, Inc offers care for private pay individuals also.
